Job Summary
Support the Social Services Director by coordinating resident services, assisting with admissions, transfers, and discharge planning; maintain documentation and navigate residents and families to community resources; advocate for residents’ psychosocial well-being; collaborate with administration, nursing, rehabilitation, and other departments; participate in policy development, quality improvement, and interdisciplinary meetings; ensure confidentiality and compliance with PHI and healthcare regulations; contribute to planning, implementing, and evaluating social service programs and staff training.
Required Qualifications
- Experience in social services, healthcare, skilled nursing, case management, or a related field preferred.
