Job Summary
In this Social Services Assistant role at a Skilled Nursing Facility, you will support the Director of Social Services in delivering resident-centered care and developing social services programs. Responsibilities include planning, implementing, and evaluating social services programs; providing counseling, advocacy, and support to residents and their legal representatives; identifying medical-related social needs and delivering appropriate services; maintaining accurate documentation related to residents’ social service needs and care plans; collaborating with the Director and interdisciplinary team; assisting residents facing personal, emotional, or environmental challenges; and ensuring compliance with federal and state regulatory requirements related to social services.
Required Qualifications
- Bachelor’s Degree in Social Work preferred; Bachelor’s Degree in Human Services or related field will be considered
- Previous experience in social services required
- Prior long-term care or skilled nursing experience strongly preferred
- Must possess any certifications or licensure required by state regulations
