Job Summary
Assist the Social Services department in a post-acute care facility to meet medically related social and emotional needs of patients in accordance with laws, regulations, and Life Care standards. Support social workers with patient and family education, liaison with community agencies, and financial planning assistance. Ensure excellent customer service, maintain confidentiality, follow company policies and compliance training, participate in an interdisciplinary team, and assist in patient evacuation. Demonstrate dependability, good judgment, and professionalism in all duties while adhering to reading/writing/speaking English requirements.
