Social Services Assistant

Coordinate social services programs by assisting the Social Service Director with planning, developing, organizing, implementing, and evaluating facility services. Meet with administration, medical, and nursing staff to direct program initiatives and participate in discharge planning, care plan development, and resident assessments. Encourage residents and responsible parties to engage in care plan meetings and assist in arranging transportation to medical appointments and post-discharge needs. Maintain records and ensure compliance with state regulations regarding medically related social services for residents.
