Social Services Assistant (FT)

Job Summary
Provide counseling, support, and advocacy to residents, assist in planning and implementing a comprehensive Social Services program, identify medical-related social needs, maintain documentation related to residents’ social service needs and care plans, collaborate with the Director of Social Services and interdisciplinary team, work directly with residents and their legal representatives facing emotional or environmental challenges, and support compliance with federal and state regulatory requirements related to social services.
Required Qualifications
- Bachelor’s Degree in Social Work preferred; Bachelor’s Degree in Human Services or related field will be considered
- Previous experience in social services required
- Prior long-term care or skilled nursing experience strongly preferred
- Must possess any certifications or licensure required by state regulations
