Social Services Assistant

Identify residents' psychosocial, mental, and emotional needs and complete social history and psychosocial assessments. Work with the interdisciplinary team to promote and protect resident rights while preventing and addressing resident abuse as mandated by law. Operate independently or within a group, utilizing computer and electronic medical record knowledge to build and maintain rapport with residents and peers. This role supports the mission of improving lives through high-quality healthcare and extraordinary compassion at Complete Care at Brackenville.
