Job Summary
Assist in identifying residents' psychosocial, mental, and emotional needs by completing social histories and psychosocial assessments for new and current residents. Collaborate with the interdisciplinary clinical team to advocate for and protect residents' rights while maintaining a safe environment through mandated reporting. Support transition planning, community resources, and daily care adjustments for residents and families. Complete required documentation accurately within the Electronic Medical Record (EMR) system and perform related administrative duties as assigned by the Director of Social Services or Facility Administrator. Requires an Illinois LSW/CSW license, experience in long-term care, and proficiency with EMR systems.
Required Qualifications
- LSW / CSW Social work license in the state of Illinois
- Experience in long-term care
- Experience in discharge planning
- Experience in care planning
- Experience in resident advocacy
- Knowledge of state and federal regulations
- Proficiency with computers and Electronic Medical Record (EMR) systems
- Strong customer service background with a proven track record of positive resident, family, and community relations
- Must pass all mandatory Illinois state healthcare worker background check requirements
