Job Summary
Coordinate resident discharge planning and provide therapeutic interventions for transitions to long-term care facilities. Document psychosocial assessments and progress toward meeting emotional needs while participating in interdisciplinary quality assurance meetings. Develop facility social work policies and support residents and families in self-advocacy through groups and referrals to external agencies. Address catastrophic events and prevent resident abuse in compliance with federal, state, and local regulations. Assist in planning and directing the Social Services Department operations to ensure resident rights and psychological wellbeing.
Required Qualifications
- Bachelor's degree in a human services field (including but not limited to: sociology, special education, rehabilitation counseling or psychology)
- An individual that possess a bachelor's degree in a human services field (including but not limited to: sociology, special education, rehabilitation counseling or psychology)
Desired Qualifications
- A minimum of one year of supervised experience in mental health or human services is preferred
