Job Summary
Social Services Aide/Designee (SSA-D) supports the Social Service Department in a long-term care facility by assisting the Social Service Director to plan, develop, and organize services in accordance with regulations and facility policies. Responsibilities include helping assess resident needs, documenting assessments and care plans, conducting or contributing to MDS/CAAs, coordinating with the Interdisciplinary Team, advocating for residents, and maintaining professional relationships with families and community organizations. The role encompasses supervision and training of staff, marketing the Social Service Program to external partners, ensuring compliance with state and federal regulations, and supporting quality improvement initiatives. The SSA-D engages in ongoing education, participates in discharge planning, contributes to resident advocacy, and performs related duties as assigned. Required Qualifications
- High School Diploma required
- Two years of experience in social service/case management in the healthcare field
- Knowledge of Federal and State Long-Term Care (LTC) Social Service regulations
- Ability to read, write, and speak English and to produce concise, meaningful, clinical written reports, assessments, documentation, and care planning initiatives
- Ability to make independent decisions when warranted and to interact professionally with residents, families, staff, and external entities
- Leadership ability and willingness to work harmoniously with other personnel under the direction of the SSD
- Ability to accept and utilize supervision, consultation, and in-service training opportunities
- Experience and qualified marketing skills to effectively express the Alden’s Social Services Program to the community and external partners
- Desire to work with diverse populations requiring long-term or short-term care
- Ability to guide staff in therapeutic approaches for dementia/behavioral populations
- Empathy, compassion, and effective interpersonal skills
- Flexibility regarding working hours
- Ability to manage confidential information
- Documentation and care planning responsibilities
- MDS/CAAs assessments and care planning experience
