Job Summary
Educate clients on the ACCESS application process and available assistance programs while guiding them through eligibility determinations for Food Stamps, Medical Assistance, and Medicare Savings Programs. Assist patients with health plan enrollment, manage My ACCESS accounts, and document demographics accurately within company systems. Monitor application progress, notify clients of decisions, and provide referrals to community agencies as needed. Maintain confidential records in compliance with HIPAA regulations and ensure all interactions follow ethical standards. This role supports Community Medical Group's mission by helping individuals and families navigate essential government benefits and community resources.
Required Qualifications
- High school diploma or equivalent
- Previous customer service experience
- Proficiency with Microsoft Office applications
Desired Qualifications
- Experience working with DCF programs, ACCESS applications, or other government assistance programs
- Experience with data entry and client management systems
- Bilingual in English and Spanish or English and Creole
