Job Summary
Provides goal-oriented and person-centered social services to residents in a long-term care setting, conducting admissions assessments, coordinating with an interdisciplinary team, maintaining charting and ensuring Resident Rights are observed. Builds relationships with residents and families, assists representatives with Medicaid paperwork and appointments, and participates in Resident Care Plan meetings. Requires a high school degree or a Bachelor's degree in social work (preferred); certification as a Social Service Designee (state) and prior geriatric/social work experience are preferred. Maintains documentation and supports scheduling and transportation for residents; collaborates to address residents' needs within the scope of practice.
Required Qualifications
- High school degree or equivalent
- Bachelor's degree in social work preferred
- Certification as a Social Service Designee by State preferred
- One year experience in long-term care social service department preferred
- Previous geriatric and social work experience preferred
- Ability to perform necessary documentation in writing and electronically
