Social Service Director

Direct and manage the day-to-day operations of the Social Service department, ensuring compliance with federal, state regulations, and community policy. Supervise staff by setting job assignments, priorities, and goals while implementing policies with follow-up. Provide direct psychosocial intervention and perform resident assessments at admission, condition changes, and annually. Create, review, and update care plans and progress notes, coordinating visits with outside services like dental and optical. Assist families in coping with skilled nursing placement, illness, and the grieving process, and participate in resident councils and family orientations.
