Social Service Director

Orient residents and families to the nursing center, develop social histories and care plans identifying medically related emotional and social needs, and answer admission inquiries while providing tours. Conduct discharge planning conferences, evaluate discharge potential, and utilize resources to ensure smooth community transitions. Assist in pre-admission processes, refer residents to external agencies, and document behavioral and psychosocial status. Provide individual and group therapeutic support for adjustment, crisis, and self-help skills, while managing complaints and establishing a volunteer program. Maintain a community resource file and reinforce the facility's commitment to high-quality care. Bachelor's degree in Social Work required; long-term care and supervisory experience desirable.
