Social Service Director- Four Seasons HC

Direct and coordinate social services for residents at Four Seasons Healthcare Center, ensuring psychosocial and concrete needs are identified and met in accordance with federal, state, and company requirements. Responsibilities include orienting residents, discharge planning, implementing and updating the Resident Care Plan and Social History, maintaining records of outside referrals, coordinating residents’ financial affairs, communicating care plans to residents, families, and staff, participating in meetings and trainings, organizing Family Council, and implementing a Quality Measurement and Performance Improvement (QMPI) program; responsibilities also include consumer service, protecting resident rights, maintaining a professional work environment, and coordinating resident moves as needed.
