Job Summary
Under the direction of the Social Services Director, the Social Services Designee will assist residents and families in adjusting to placement, illness, and the plan of care, helping them attain the highest practicable level of functioning in accordance with current federal and state regulations and facility policies. Responsibilities include assisting residents and families with treatment wishes, aiding in transition and adjustment to the facility, participating in resident and family council meetings as requested, and supporting admissions and discharge planning services. Requires a high school diploma or equivalent; knowledge of long-term care regulations; strong communication and confidentiality; and the ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals.
Required Qualifications

- Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals
- Ability to organize, prioritize, and meet deadlines
- Ability to maintain confidentiality and be discreet
- Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care
