Job Summary
The Social Services Designee will assist residents and families in adjusting to placement and care plans, helping them achieve the highest possible level of functioning. Responsibilities include supporting residents and families with treatment wishes, participating in council meetings, and assisting with admissions and discharge planning. Required qualifications include a high school diploma or equivalent, the ability to work with diverse elderly populations, and knowledge of nursing regulations. Preferred qualifications include experience with geriatric populations and a degree in a related field.
Required Qualifications
- High school diploma or equivalent
- Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals
- Ability to organize, prioritize, and meet deadlines
- Ability to maintain confidentiality and be discreet
- Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care
Desired Qualifications
- Experience with geriatric population
- AA or Bachelor's degree in a related field
- Creative problem-solving skills
- Strong communication skills
- Sufficient computer skills to manage records and documentation
