Job Summary
Coordinate resident discharges, assist in preparing residents for their future, and ensure social and emotional needs are met during their stay. Participate in resident care conferences to maintain high standards of care and ensure resident comfort and safety. We seek a candidate with a high school diploma and two years of social worker experience, with a bachelor's degree and five years of related experience preferable.
Required Qualifications

- experience in a healthcare setting
