Job Summary
Assist each resident and family in adjusting to placement, illness, and the plan of care to help them attain the highest practicable level of functioning under current federal, state, and facility policies. Support residents and families with wishes regarding treatment and aid their transition and adjustment to the facility. Participate in resident and family council meetings as requested and support Admissions and discharge planning services. Work with physically and mentally ill, disabled, and emotionally upset elderly individuals while maintaining confidentiality and adhering to nursing practices and long-term care regulations.
Required Qualifications
- High school diploma or equivalent
- Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals
- Ability to organize, prioritize, and meet deadlines
- Ability to maintain confidentiality and be discreet
- Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care
Desired Qualifications
- Experience with geriatric population
- AA or Bachelor's degree in a related field
- Creative problem-solving skills
- Strong communication skills
- Sufficient computer skills to manage records and documentation
