Job Summary
Ensure residents' psychosocial and concrete needs are identified and met per federal, state, and company requirements. Orient new residents, conduct discharge planning, and implement and update Resident Care Plans and Social Histories. Maintain records of outside referrals, coordinate financial affairs, and communicate care plans to families and staff. Participate in meetings, trainings, and Family Councils as requested. Organize resident room moves and maintain an ongoing Quality Measurement and Performance Improvement Program. Present a professional image while protecting resident rights and adhering to company standards for resolving concerns. Requires a Bachelor's in Social Work or human services (LSW required in TX Centers) with one year of supervised healthcare experience; high school diploma sufficient for centers under 120 beds.
Required Qualifications
- A Bachelors Degree in Social Work or a human services field (LSW required in TX Centers)
- one year of supervised work experience in a health care setting working directly with individuals
- High School Diploma or equivalent
