Job Summary
Develop and implement communal and individual opportunities to support tenants aging in place within senior housing properties. Facilitate coordinated care for high-need cases, conduct apartment visits, and respond to emergency situations while maintaining contact with service providers. Provide information and referral services, advocate with community providers, and assist tenants with applications and recertification. Work through a team approach with property management to identify and respond to tenant needs. Ensure educational and social programming occurs consistently throughout the year. Accompany tenants to meetings as requested. Requires a Bachelor's degree, flexible evening/weekend hours, and ability to travel throughout the five boroughs.
Required Qualifications
- Bachelor's Degree in Human Services or related field
- Excellent organizational, verbal, & written communication skills
- Ability to work flexible hours (evenings/weekends) as assigned
- Able to travel throughout the five boroughs as needed
- Ability to lift up to 10lbs
