Social Service Case Manager

Social Service Case Manager providing in-home visits across seven counties to monitor safety, connect clients with community resources, and support independence. Responsibilities include managing a caseload with compassionate, quality service; advocating for clients; collaborating with a values-driven team (Caring, Curiosity, Trust, Excellence); and traveling across the service area with reliable transportation. Key skills include strong computer and written/verbal communication, sound judgment, organization, and the ability to navigate complex situations. Minimum requirement of a Bachelor's degree in a related field and a valid driver’s license with current auto insurance. Starting pay is $21.45/hour.
