Social Service Assistant

Conduct psychosocial assessments and document progress for residents to address emotional and psychological needs. Participate in interdisciplinary team meetings to review facility policies, ensure regulatory compliance, and promote resident rights. Provide therapeutic interventions for transitions, support families in self-advocacy, and coordinate discharge planning with referrals to external agencies. Deliver groups and clinical interventions for catastrophic events while preventing and addressing resident abuse.
