Job Summary
The Social Care Navigator will support community members facing challenges by connecting them to resources addressing their social needs and improving their health. Responsibilities include identifying unmet social needs through screening, completing eligibility assessments, and providing navigation services. Candidates are expected to have computer skills, experience working in collaborative environments, and the ability to manage various responsibilities independently. The position offers a temporary employment opportunity with benefits, including health insurance and retirement.
Required Qualifications
- Computer skills including use of e-mail and computer based time collection
- Willingness to learn new referral management platforms (primarily Unite Us)
- Ability to work independently with a minimum of supervision
- Experience working in a team-oriented, collaborative environment
- Ability to rapidly adapt and respond to changes in environment and priorities
- Ability to identify and address safety concerns in the workplace
- Ability to work effectively with co-workers
- Ability to follow oral and written instructions
- Ability to plan and organize work responsibilities according to priorities
- Ability to meet travel requirements associated with this position
- Ability to work flexible hours including evenings and/or weekends
Desired Qualifications
- Experience with community members facing challenges
- Familiarity and interest in 1115 Waiver goals and procedures
- Familiarity with trauma-informed approaches
- Interest in advancing health equity through cross-sector collaboration and health and social care integration
- Experience managing a caseload of clients and providing 1:1 care management support
