Job Summary
Screen Medicaid members for Health Related Social Needs (HRSN), conduct outreach, eligibility assessments, and referrals, and coordinate care with community agencies and healthcare providers. Develop Social Care Plans summarizing member needs and service referrals, while monitoring communications via phone or the Care Compass website. Assist in mentoring Level 1 Navigators and facilitate meetings in person or virtually using Microsoft Office tools. Maintain HIPAA compliance and collect data on member outcomes to inform program improvements. This role requires a high school diploma or equivalent plus two years of healthcare or social services experience, familiarity with the U.S. healthcare system, and strong communication skills. Candidates must reside within two hours of Binghamton, NY, and work in a hybrid model with occasional travel for partner meetings.
Required Qualifications
- All applicants must reside within 2 hours of Binghamton, NY
- Possession of a high school diploma or equivalent
- two (2) years of work experience in the health care, community health, or social services field
- two (2) years of college-level training in health occupations may substitute for one (1) year of work experience
- Familiarity with the U.S. healthcare system and federally funded healthcare programs
- Possesses strong communication skills (written and verbal)
- Negotiation skills and the ability to develop strong working relationships
- Proficient in planning & time management
- Ability to work independently with minimal supervision
- Utilizes critical thinking skills
- Possesses adaptability skills (both in attitude and action)
- Must have strong personal computer program skills including Microsoft Outlook, Microsoft Office (Word, Excel PowerPoint etc.)
- Facilitation of meetings either in person or via virtual platforms such as Zoom
- MUST Maintain HIPAA compliance at all times
