SOAR Benefits Specialist

SOAR Benefits Specialist to increase access to income and benefits for adults transitioning out of nursing facilities. Primary responsibilities include evaluating client eligibility for entitlements, maintaining a caseload of clients applying for benefits, gathering medical history, submitting SSI/SSDI benefit applications via the SOAR process, entering data into the SOAR system, coordinating with Social Security staff, assisting with Medicaid spenddown and other benefits (e.g., LINK/food stamps), explaining representative payee services, accompanying clients to appointments and hearings, and ensuring completion of required documentation. The role requires completing SOAR training within 45 days of hire, achieving productivity targets (approximately 50% direct service), maintaining compliance with center policies and state funder requirements, and participating in statewide learning collaborations. Work environment includes community-based duties with possible Work from Home portions upon approval; travel to nursing facilities and varied residential settings is required.
