Job Summary
SNP Lead Care Manager role in Peak Health's SNP program leading care management strategies. Responsibilities include onboarding and mentoring care management staff, performing documentation audits, coaching and education, ensuring CMS SNP Model of Care (MOC) and NCQA standards compliance, policy/procedure development for care management, monitoring performance and staff coverage, delivering education strategies, data collection and trend analysis, escalation handling for high-risk cases, and collaboration across the Interdisciplinary Care Team (ICT) and Peak Health partners. The role requires strong clinical leadership, ability to present complex data to diverse stakeholders, and liaison work between SNP Care Management, providers, pharmacy, behavioral health partners, and community agencies. Travel may be required; standard 40-hour work week; exempt status.
Required Qualifications
- Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC)
- Five (5) years of healthcare clinical experience
- Two (2) years’ Care Management, Case Management or Population Health experience
- Bachelor's Degree in Nursing OR Associate of Science in Nursing Degree (ASN) or Diploma (preferred)
- Experience: Management of Medicare and/or Medicaid and/or SNP populations (preferred)
- One (1) year SNP Care Management experience (preferred)
