SNP Case Manager RN

Coordinate and optimize care for SNP beneficiaries as an RN Case Manager, including initial and ongoing assessments, chart reviews, and participation in case conferences; collaborate with the Program Director to review and develop individualized care plans; advocate for patients to access medical services, entitlements, and psychosocial resources; ensure timely and accurate patient information for reporting; liaise with primary care providers, specialists, and care teams; promote access to community resources, identify high-risk patients, and support care transitions through coordinated interventions and data collection; engage in outreach to promote program engagement and improve care coordination; accountabilities include medication reconciliation at discharge and guiding patients through DME and home health services as needed.
