Job Summary
The SNF LVN supports Quality Assurance operations by managing data/metrics, auditing discharge charts, coordinating labs/imaging, and handling referrals (home health, hospice, dermatology, PCP). Responsibilities include maintaining a provider/patient issue spreadsheet, conducting clinical audits on active patients, monitoring wound cultures and antibiotic prescriptions, and uploading lab results to patient charts, with strong emphasis on communication and collaboration across departments.
Required Qualifications
- Current California LVN
- 1-2 years SNF experience preferred
- Strong leadership skills
- Strong written and verbal communication skills
- Excellent collaboration and product management skills and experience
- Knowledge of safety requirement and legal standards
