Job Summary
Conduct comprehensive speech-language evaluations and assess home environments as appropriate. Develop and update individualized plans of care in collaboration with patients, caregivers, and the care team. Provide speech-language therapy services as ordered by the physician to identify functional deficits and implement targeted treatment plans. Utilize specialized techniques to improve patient outcomes, educate families, and maintain accurate clinical documentation. Participate in case conferences, supervise home health aides, and make referrals as needed. This role offers a flexible schedule to balance work and life while supporting meaningful communication and safety improvements for residents in the Cortez and Grand Junction area.
Required Qualifications
- Master's or doctoral degree in Speech‐Language Pathology
- Current licensure to practice in the State of Colorado as a Speech‐Language Pathologist
- Minimum 1 year of clinical experience in Speech‐Language Pathology
- Strong verbal, written communication, and organizational skills
- Current CPR certification
- Valid driver's license, automobile insurance, and reliable transportation for patient visits
