Job Summary
Oversee the person-centered program planning through supervision, development, and management of client services; supervise and train providers; ensure regulatory compliance and documentation in Electronic Medical Record; coordinate services for home placements; conduct monthly home visits; perform medication administration as assigned; collaborate with the Shared Living Coordinator to develop and supervise individual home share placements; ensure adherence to state funding, certification regulations, and quality service standards; coordinate service planning meetings and referral/intake processes; maintain strong communication and organizational practices to support individuals with developmental delays, disabilities, or family challenges.
Required Qualifications
- High School diploma or GED
- Minimum of one year related work experience
- Must be 18 years of age
- Valid driver's license in good standing
- Successful clearance of local and/or state background checks
- Excellent communication skills with an ability to establish rapport with team members and those we serve
- Strong organizational abilities to ensure staffing and schedules are maintained
- Supervisory experience preferred
