Job Summary
Speech Language Pathologist (SLP) provides intermittent speech therapy services in patients’ residences under physician orders and plan of care, supervised by Clinical Manager/Therapy Supervisor. Responsibilities include assisting evaluation of functional level, developing and revising the plan of care, preparing clinical and progress notes, advising families and agency personnel, participating in staff in-service programs, completing initial history/evaluation, and delivering all skilled SLP services as ordered by a physician. Requires a master’s (or doctoral) degree in speech-language pathology and state licensure or approved alternate pathway, with two years of experience preferred. Reports to Clinical Manager/Therapy Supervisor; interacts with physician and other health care personnel; ensures adherence to policies and procedures.
Required Qualifications
- Master's or doctoral degree in Speech-Language Pathology
- State licensure as Speech-Language Pathologist OR meets alt licensure pathway with practicum/experience and national exam as applicable
