Job Summary
Independently licensed therapists (LCSW, LMFT, or CMHC) are sought to provide diagnostic evaluations, psychotherapy, and treatment planning in SNF/LTC facilities in the Salt Lake City area. Collaborates with psychiatrists, psychologists, care coordinators, and facility staff; completes documentation in the EMR (Athena I) remotely; ensures compliance with agency policies. Benefits include base salary with incentives, PTO, CEU allowances, and a comprehensive benefits package; flexible scheduling with four weeks PTO, holidays, and 401(k). Fluency in Spanish is a plus. Requires Utah licensure as LICSW/LMFT/LPC and an active NPI; Medicaid/Medicare credentialing is advantageous.
Required Qualifications
- Master of Social Work, Counseling, or other eligible master's degree
- Utah licensure as an LICSW, LMFT, or LPC
- NPI (National Provider Identifier)
- Credentialed with Medicaid/Medicare a PLUS
- TB Test
- CPR/AED/First Aid Certification
