Skilled Nursing LPN

Licensed Practical Nurse (LPN) provides routine care, observes patients’ health, assists doctors and registered nurses, and communicates instructions to patients regarding. Supervises and delegates tasks to the nursing assistants or aides who work under their direction. Evaluates and reports changes in residents' physical or mental condition to the appropriate personnel and maintains residents’ medical records in a confidential manner. Responsible for direct care and/or assisting with care, initiates SBAR, collaborates with the RN for assessment and recommendations, participates in care planning and evaluations, coordinates care activities, oversees Medication Aide/Tech for medication/treatment administration, administers treatments, and ensures documentation and communication per regulations. Includes a $6,000 sign-on bonus for full-time roles and a benefits package.
