Skilled Nursing Facility Coordinator

Job Summary

Coordinate communication between Skilled Nursing Facilities, patients, providers, and care teams via phone, email, fax, Docusign, and text. Serve as the central point of contact to support timely information sharing, align clinician schedules, and track outstanding documentation and follow-up items. Navigate insurance requirements, maintain accurate patient records, and monitor referrals to ensure continuity of care. Build relationships with SNF partners across the Chicago area while maintaining a consistent on-site presence of 2-3 days per week. Collaborate with facility staff to obtain clinical and administrative documentation and identify referral opportunities. Key 30 words per minute with accuracy and use the Alpha filing system and Windows-based software.

Required Qualifications

  • High school diploma or equivalent combination of education and experience
  • At least two to four years of customer service, administrative, or related experience
  • Experience in a receptionist position
  • Experience with medical front office procedures
  • Valid driver's license
  • Driving record within the standards outlined within Hanger's Motor Vehicle Safety Policy and Procedures
  • Strong interpersonal, oral (including telephone) and written communication skills
  • Ability to key 30 words per minute with accuracy
  • Ability to work with handicapped individuals
  • Understanding of Alpha filing system
  • Understanding of medical reimbursement and terminology
  • Understanding of front office (receptionist) duties
  • Ability to use a computer, including Windows-based software and e-mail
  • Ability to use all necessary office equipment, facsimile machines, calculator, postage machine, copier, etc.

Desired Qualifications
