Job Summary
Coordinate and manage daily activities for Skilled Nursing Center patients and residents under the supervision of the Administrator and a Registered Nurse. Develop care plans that accommodate services across the lifespan while adhering to hospital policy, mission statements, and the Department of Nursing philosophy. Deliver direct patient care including vital signs monitoring and activities of daily living, utilizing proficiency in data entry for accurate care documentation. Maintain certification as a Texas Nursing Assistant and Basic Life Support, while demonstrating strong communication and organizational skills within the health care team. Work full-time days at OakBend Medical Center's skilled nursing facility, supporting the team's commitment to compassionate community care since 1950.
Required Qualifications
- High school graduate or GED equivalent
- Previous experience working with aged or handicapped individuals
- Certification in the State of Texas as a Nursing Assistant
- Basic Life support Certification through the American heart Association
- Certified Activities Director or currently enrolled in state approved activities director course
- Knowledge of basic clinical skills to deliver direct patient care to an individual or group of patients including but not limited to vital signs and activities of daily living
- Accepts assigned responsibilities as an accountable member of the health care team
- Proficient with data entry/computer skills for care documentation
- Excellent communication & organizational skills

- Experience in an accredited healthcare facility
