Job Summary
Collaborate with therapists to develop, implement, and revise treatment plans for patients and families. Deliver individual and group skills training covering self-care, communication, illness management, and problem-solving. Provide case management services and coordinate care by working with medical providers and external agencies. Complete CANS/ANSA reassessments and maintain accurate clinical documentation while serving as a liaison between patients and institutions. Requires a high school diploma, two years of behavioral healthcare experience, or equivalent coursework; associate degree preferred. Valid driver's license, reliable transportation, and auto insurance are mandatory. Required Qualifications
- High school diploma or its equivalent
- 2 years behavioral healthcare experience
- Equivalent college coursework toward a human services or health related degree
- Valid driver's license
