Conduct advanced home care for adult patients in Innerstaden, managing physical, psychological, social, and existential needs while ensuring patient and family participation in care. Document progress in the Take Care journaling system and plan treatment using the Alfa Welfare tool. Provide independent nursing duties within a collaborative team environment, requiring strong clinical judgment and the ability to make autonomous decisions in patient-proximal situations. Work during daytime hours from our base at Linnégatan, Östermalm, with access to electric bicycles for staff. A valid B license is mandatory.
