Siouxland Pathway Case Manager

Job Summary
Conduct intensive, face-to-face case management for participants in Woodbury County, meeting weekly to assess needs, set Individual Career Plans, and remove barriers to employment. Screen for mental health and substance abuse issues, refer to community partners, and facilitate enrollment in vocational training and Job Training Partners. Maintain accurate records, input case notes into the data system, and submit quarterly reports per DOL REO grant requirements. Collaborate with Iowa Workforce Development, employer partners, and volunteers while attending staff meetings and professional development activities. Ensure culturally responsive, inclusive services and maintain a safe, drug-free environment.
Required Qualifications
- BA or BS Degree in Social Work, Education or closely related discipline
- At least 3 years of case management experience
- Experience working with justice-involved individuals (adults or youth)
- Good written and verbal skills
- Public speaking skills
- Must be able to pass a background check to enter County or State run correctional facility
- Ability to travel throughout the service area (Woodbury County) as necessary, to monitor and assess program, young adults and program partners
- Must have a clean driving record
Desired Qualifications
- Lived experience as a formerly incarcerated or justice-involved individual
- BA or BS Degree in Social Work, Education or closely related discipline preferred
