Conduct comprehensive physical therapy evaluations in the home setting and develop individualized plans of care based on patient needs and physician orders. Provide hands-on therapy to improve mobility, strength, balance, and functional independence while educating patients and caregivers on treatment plans, exercises, and safety strategies. Accurately document visits, progress, and outcomes in the EMR system and collaborate with interdisciplinary team members including nurses, OT, SLP, and physicians to monitor patient progress and adjust treatment plans as needed.
