Job Summary
Provide home-based physical therapy to patients in Lincoln City, OR on a PRN/hourly basis. Conduct comprehensive physical therapy evaluations in the home setting, develop and implement individualized plans of care, and deliver hands-on therapy to improve mobility, strength, balance, and functional independence. Educate patients and caregivers on treatment plans, exercises, and safety strategies; accurately document visits, progress, and outcomes in the EMR. Collaborate with interdisciplinary team members (nurses, OT, SLP, and physicians); monitor patient progress and adjust treatment plans as needed; ensure compliance with regulatory, ethical, and company standards.
Required Qualifications
- Graduate of an accredited Physical Therapy program
- Current Oregon Physical Therapist license (or ability to obtain)
