Job Summary
Home Health Medical Social Worker (MSW) with Part Time/PRN status in Eugene, OR collaborates with patients, families, and the health-care team to implement care plans, conduct psychosocial assessments, provide counseling, education, and referrals, participate in discharge planning, supervise SW Assistants as directed, and contribute to quality improvement efforts. Responsibilities include developing individualized plans of care, communicating psychosocial findings to the care team, maintaining clinical notes, coordinating with community resources, and supporting organizational care goals within a home health setting. Requires MSW from an accredited program, CPR certification, driving access, and applicable licensure; prior health-care social work experience preferred.
Required Qualifications
- Master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education
- Minimum of one year's social work experience in health care setting
- Possesses and maintains current CPR certification
- Valid driver’s license and access to automobile for travel related to agency activities
