In-Home Health Evaluations for Nurse Practitioner or Physician Assistant: travel-focused, full-time clinician role performing 45-minute, in-home health evaluations using an iPad and a defined clinical workflow. Responsibilities include patient engagement, medical history review, physical examination, and, when ordered by the health plan, additional diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screening). Clinicians travel across a defined coverage area (up to 100% travel), may involve overnight stays, and must maintain licensure in multiple states as required. This role emphasizes building trusted relationships, delivering comprehensive and personalized health evaluations directly at patients’ homes, with virtual visits as needed; no prescribing or altering current treatment regimens unless directed by the patient’s health plan.
