Job Summary
Conduct full-time in-home health evaluations across defined coverage areas, including Allentown, Altoona, and Philadelphia, while traveling up to 100% of the time with overnight stays as needed. Perform 45-minute visits featuring medication reviews, physical evaluations, and diagnostic tests like spirometry and diabetic eye exams using an iPad clinical workflow. Hold active, unrestricted multi-state medical licenses to serve members in diverse home environments that may contain pets or allergens. Engage in virtual visits as required and collaborate with Signify Health's concierge activation team to manage licensure in new states. This role supports a national network of clinicians dedicated to bridging care gaps and providing personalized health services to millions of people nationwide.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Holding multiple state licenses
- Be flexible to travel locally and within licensed states
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
- Must be able to travel up to 100% of the time
- Must be able to accommodate overnight hotel stays, car rentals and/or flights
