Job Summary
Provide in-home health evaluations across a defined New York coverage area, primarily traveling to patients’ homes (up to 100% of the time) with 45-minute visits that include medical history review, physical evaluation, and potential diagnostic tests; use an iPad and a clinical workflow; must hold active licenses and board certification, with ability to obtain/maintain multi-state licenses; NY coverage includes NYC and multiple counties, with potential travel to other states as needed; sign-on bonus eligible and full-time benefits package available.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover the following primary locations: Able to cover all counties within New York and flex to other areas of business need
- New York State Coverage Areas Include: Albany, Binghamton, Buffalo, Elmira, NYC, Plattsburgh, Poughkeepsie, Rochester, Syracuse, Utica and Watertown
- Signify Health/ CVS Health affiliation with in-home clinician role
- In-home visits up to 100% travel across defined coverage area
- Utilize iPad and clinical workflow for evaluations
- 45-minute visits including medical history, physical evaluation, and medication review
- No required prescribing of medicine or labs during visits
- Support for licensure in multiple states via Network Activation team
- Benefits eligibility for full-time employees
