Job Summary
Conduct in-home health evaluations across New York, engaging with patients in their private residences for 45-minute visits. Perform medication reviews, physical examinations, and administer diagnostic tests such as spirometry and diabetic eye exams using an iPad clinical workflow. Manage regions within licensed states including NY, ME, MA, RI, or CT, with up to 100% travel time requiring overnight stays and air or ground transportation. Perform virtual visits as needed while adhering to safety protocols in diverse home environments. Receive per-completion incentives for Diagnostic and Preventive Services and base pay for each evaluation completed.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within New York
- Licensed in one or more of these states: NY, ME, MA, RI, or CT
- Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
- Must be able to travel locally and within licensed states, up to 100% of the time
- Must be able to obtain and maintain multi-state licenses in coverage area, as required
