Job Summary
Conduct in-home health evaluations across New Hampshire, engaging with patients in their residences for 45-minute visits that include medication reviews, physical exams, and diagnostic tests like spirometry. Travel up to 100% of the time within defined coverage areas, which may require overnight stays and adherence to multi-state licensing requirements. Perform virtual visits as needed while utilizing an iPad-based clinical workflow to provide comprehensive preventive and diagnostic services. Work as a clinician colleague within Signify Health's national network, focusing on building trusted relationships and guiding patients toward better health outcomes without prescribing or altering treatment regimens.
Required Qualifications
- Primary residence in the state of New Hampshire
- Full-time employment
- In-home health evaluations
- Engaging with people in the comfort of their own homes
- Up to 100% travel across a defined coverage area
- Overnight hotel stays, car rentals and/or flights
- Holding multiple state licenses
- Active, unrestricted license(s) in coverage area(s)
- Board certification
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within New Hampshire
- Flexibility to travel locally and within licensed states, up to 100% of the time
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products
- Comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
