Job Summary
Full-time in-home clinician role providing in-home health evaluations for health plan members in New Hampshire, traveling up to 100% across defined coverage areas. Conduct 45-minute visits including medication and medical history review, physical evaluation, and potential diagnostic tests (e.g., spirometry) as ordered by health plans. Use an iPad and clinical workflow; not required to prescribe medications, order labs, or alter current treatment. Role requires active, unrestricted NP or PA license, board certification, and ability to obtain/maintain multi-state licenses; flexibility to travel locally and within licensed states; primary workplace is the member’s home; counties in NH + potential expansion as business needs dictate. Competitive benefits and sign-on bonus eligibility; weekly hours around 40. Anticipated coverage includes NH counties such as Rockingham, Strafford, Belknap, Merrimack, Sullivan, Coos, Carroll, Grafton, Cheshire, Manchester, and Hillsborough; additional licensure support provided by Network Activation team.

- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover the following primary locations: Able to cover all counties within New Hampshire and flex to other areas of business need
- NH Coverage Market Areas include: Rockingham, Strafford, Belknap, Merrimack, Sullivan, Coos, Carroll, Grafton, Cheshire, Manchester and Hillsborough
