Job Summary
Conduct in-home health evaluations with patients, performing medication reviews, physical assessments, and diagnostic tests like spirometry and diabetic eye exams using an iPad-based clinical workflow. Travel up to 100% of the time across defined coverage areas, including overnight stays and flights, while conducting virtual visits as needed. Provide comprehensive preventive and diagnostic services directly in private residences, ensuring safe and comfortable interactions with household pets, smoke, and allergens. Earn additional per-completion incentives for each visit completed.
Required Qualifications
- Full-time Clinician (Nurse Practitioner or Physician Assistant)
- Active, unrestricted license(s) in coverage area(s)
- Board certification
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within [state] and flex to other areas of business need
- Be flexible to travel locally and within licensed states, up to 100% of the time
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.)
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
