Job Summary
Conduct in-home health evaluations across Maine, performing medication reviews, physical exams, and diagnostic tests like spirometry and diabetic eye exams during 45-minute visits. Travel up to 100% of the time, including overnight stays and flights, to cover all counties within Maine and flex to other areas as business needs dictate. Perform virtual visits as needed while using an iPad clinical workflow for each assessment. Engage directly with patients in their residences to bridge care gaps and provide personalized health education.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within Maine and flex to other areas of business need
- Primary residence in the state of Maine
- Full-time employment
- Travel up to 100% of the time
- Ability to conduct overnight hotel stays, car rentals and/or flights
- Holding multiple state licenses
- Comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
