Job Summary
In-Home Clinician role for Nurse Practitioner or Physician Assistant performing in-home health evaluations across defined coverage areas. Travel up to 100% with potential overnight stays; primary residence in MA required. Role includes conducting 45-minute in-home visits with health-question discussions, medication and medical history review, physical evaluation, and optional diagnostic tests as ordered by health plans. Use of an iPad with a clinical workflow; no prescribing or altering current treatment during visits. Must hold active licenses and board certification, and be able to obtain/maintain multi-state licensure as needed. Full-time position with benefits; signing bonus eligible. Responsibilities emphasize establishing trusted relationships to connect patients with appropriate care, and flexibility to travel locally and to other states as required by business needs. Coverage currently includes MA counties (e.g., Barnstable, Dukes, Nantucket, etc.) with potential expansion. Anticipated weekly hours: 40.
